Consent to Telehealth

Acknowledgment

By accessing this website, its associated applications, platform, and/or services, you acknowledge that you are over the age of 18 and have read, understood, and agree to be legally bound by the terms set forth herein. If you do not or cannot agree with any part of this Consent to Telehealth, you may not access or use the website, applications, platform, or services.

Introduction

MAT’S CLINIC (“we,” “our,” “us,” or “MAT’S CLINIC”) provides access to a website and related platform through which users (“you” or “your”) may request and obtain online consultations with affiliated, independent, licensed physicians, nurse practitioners, and other healthcare providers (“Providers”). These Providers are contracted through Architek Health Provider Network, P.C. (the “Medical Group”).

Services may include medical consultations, treatment plans, diagnoses, prescriptions, and coordination of at-home laboratory testing through partnered pharmacies and service providers, including Boudreaux’s New Drug Store. These services are delivered via telehealth, which may include synchronous video or audio communication, asynchronous messaging, secure chat, SMS, email, or other electronic means.

Telehealth may involve the electronic transmission of personal health information, including medical records, laboratory results, images, video, or data obtained from remote monitoring devices, for purposes of evaluation, diagnosis, treatment, and follow-up.

Potential Benefits of Telehealth

Telehealth services may offer the following benefits:

  • Access to medical care without requiring in-person visits
  • Greater convenience and scheduling flexibility
  • Increased privacy and efficiency
  • Potentially improved continuity of care and reduced costs

Potential Risks and Limitations of Telehealth

Telehealth involves certain risks and limitations, including but not limited to:

  • Technology failures or interruptions
  • Miscommunication between you and your Provider
  • Unauthorized access to electronic communications
  • Incomplete information that may limit diagnostic accuracy

Telehealth may not be appropriate for all medical conditions. Regulatory requirements, your medical history, or your presenting condition may limit access to certain services or prescriptions.

Primary Care Responsibility

Providers accessed through MAT’S CLINIC do not replace your primary care physician. You are encouraged to maintain an ongoing relationship with a local primary care provider and other specialists as appropriate.

Telehealth is not for emergencies.
If you experience a medical emergency, call 911 or go to the nearest emergency department immediately.

No Guarantee of Results

There is no guarantee that the use of MAT’S CLINIC services or products will result in a desired or positive outcome. Medical outcomes vary, and all treatment decisions should be discussed with your Provider. You must promptly notify your Provider of any adverse or unexpected effects.

Duty to Update Information

You agree to notify your Provider of any changes that may affect your eligibility for care, including pregnancy, breastfeeding, new diagnoses, medication changes, or treatments received elsewhere. You are responsible for providing complete and accurate medical information.

Referrals

Certain conditions may require in-person evaluation. Providers may refer you to a local healthcare professional or emergency facility when telehealth is not clinically appropriate.

Insurance and Financial Responsibility

MAT’S CLINIC does not accept insurance. All services are self-pay. We do not participate in Medicare, Medicaid, or Tricare. You are financially responsible for all services rendered.

  7. Agree to provide truthful and complete medical information
  8. Consent to the collection, use, and sharing of your information as permitted by law
  9. Agree to hold harmless MAT’S CLINIC and affiliated entities for delays or interruptions caused by technology failures
  10. Understand consent may be withdrawn in writing at info@matsclinic.com
